Metabolic Profile and Cardiovascular Risk in a Population of Renal transplant recipients
Introduction. Cardiovascular disease is more common in renal transplant recipients
(RTRs) than in the general population, and is the major cause of both graft loss and patient death in RTRs.
Objectives. This study aimed to characterize the cardiovascular risk factors, calculate the
7-year risk for major adverse cardiac events and the 7-year risk for death in a population of RTRs using a cardiovascular risk calculator, and determine the main cardiovascular risk factors associated with increased prediction of major adverse cardiac event (MACE) and death. Patients. This is a retrospective review of clinical data from 121 RTRs who are in follow-up programs at our institution, and who had a functioning and stable graft for longer than 6 months.
Results. Among 121 adult patients followed at our institution (59.5% males, mean age of
49.6 13.8 years, mean times for functioning grafts were 105 73.5 mo), 86.8% had
hypertension, 19.8% had diabetes, 24.8% were current or former smokers, 61.9% had
increased body mass index, and 71% had dyslipidemia. The 7-year risk for MACE was
more than 10% in 38 (31.4%) patients with age, diabetes, and smoke being independent
risk predictors. The 7-year risk for death was more than 10% in 56 (46.3%) patients
with age, diabetes, blood pressure, smoking, and male gender being independent risk
predictors.
Conclusion. There is a high prevalence of cardiovascular risk factors in a population of
RTRs, and there is increased risk for MACE and death. Accurate risk prediction is
important for physician decision support and patient education, promoting improved
cardiovascular health of RTRs, and thus prolonging the survival of both patients and graft.info:eu-repo/semantics/publishedVersio
Giant cell glioblastoma: review of the literature and illustrated case
Giant cell glioblastoma is an infrequent variety of glioblastoma (5% of the cases). It has deserved a separate category in the World Health Organization classification of grade IV tumors. The clinical, imaging, histological and immunohistochemical characteristics, and the genetic alterations are reviewed. Treatment and prognosis are discussed and updated. The case of a patient that survived 19 months and died of spinal leptomeningeal metastases is illustrated
Occupational participation, stress, anxiety and depression in workers and students from Brazilian universities during the COVID-19 pandemic
Abstract Introduction The physical, social and occupational restrictions imposed by the COVID-19 pandemic have affected the health and well-being of the world population. Objective To identify the repercussions of the pandemic on the occupational participation of students, lecturers and technicians from three public universities in Northern Brazil, to compare the changes reported by participants in occupational participation before and during the pandemic, and to identify symptoms of depression, anxiety and stress self-reported. Method This is a Cross-sectional, descriptive and comparative study with a quantitative approach. One hundred and ninety-nine (n = 199) participants (students, lecturers and technicians) responded to an online questionnaire, the “Occupational Participation Checklist” and the Anxiety, Stress and Depression Scale (DASS-21). Data analysis were descriptive and also performed by applying the Wilcoxon and Mann-Whitney tests. Results During the pandemic, an increase in occupational participation was identified for all participants in domestic activities (p <0.001) and a decrease in work and study face to face (p <0.001). Students reported more symptoms of depression, anxiety and stress when compared to lecturers (p<0.001). Most students did not organise their time to fulfil their occupations with satisfaction. Such difficulties were associated with symptoms of depression, anxiety and stress, especially among the student's group (p<0.001). Conclusion This study provided preliminary evidence about differences in occupational participation before and during the Covid-19 pandemic. The organization of time and difficulties in occupational participation were associated to levels of anxiety, depression and stress, especially in the sample of students
Tailoring Three-Point Functions and Integrability II. Weak/strong coupling match
We compute three-point functions of single trace operators in planar N=4 SYM.
We consider the limit where one of the operators is much smaller than the other
two. We find a precise match between weak and strong coupling in the
Frolov-Tseytlin classical limit for a very general class of classical
solutions. To achieve this match we clarify the issue of back-reaction and
identify precisely which three-point functions are captured by a classical
computation.Comment: 36 pages. v2: figure added, references adde
Predictors of response to TNF blockers in patients with polyarticular psoriatic arthritis
Psoriatic arthritis (PsA) is a chronic inflammatory
rheumatic disease with a broad clinical spectrum. PsA
can affect the axialskeleton, peripheral joints, entheses,
synovial sheaths of tendons, skin, nails and extra-articular
organs. Tumour necrosis factor alpha blockers
(TNF blockers) were a breakthrough development in
the treatment of PsA. Identifying predictors of response
to biological therapiesin patients with PsA is of utmost
importance, especially in view of the costs and potential
side effects of these agents. The aims of the present
study were to determine baseline predictive factors of
response to biological therapies, at 3 and 6 months, in
PsA patients with polyarticular involvement (with or
without axial involvement). Data were collected from
the RheumaticDiseases Portuguese Register(Reuma.pt).
Eligible patients had to be anti-TNF-naive at baseline
and to have at least 3 months of follow-up after the beginning
of TNF blocker therapy. Only patients with information
on at least one of the response measures (at
3 or 6 months of follow-up) were included in the analysis.
Univariable logistic regression analysis of potential
baseline predictors of European League Against Rheu-matism (EULAR) good clinical response, EULAR good/
/moderate response, 28-joint Disease Activity Score
with three variables including the erythrocyte sedimentation
rate (DAS28-3V-ESR) remission and Health
Assessment Questionnaire (HAQ) response were performed.
Multivariable logistic regression using a forward
selection procedure was used until the best-fit
model was obtained, taking confounding effects into
account. A total of 180 patients were eligible for the
study (mean age 52 years, 54% women). In multivariable
analysis at 3 months, females were less likely to attain
a good EULAR response [OR=0.082 (95%
CI=0.024, 0.278)], a DAS28-3V-ESR remission
[OR=0.083 (95% CI=0.017, 0.416)], a moderate or
good EULAR response [OR=0.091 (95% CI=0.011,
0.091)] and a HAQ response [OR=0.074 (95%
CI=0.009, 0.608)]. At 6 months, female gender was
also less likely to achieve a good EULAR response
[OR=0.060 (95% CI=0.011, 0.325)], DAS28-3V-ESR
remission [OR=0.060 (95% CI=0.012, 0.297)], and a
HAQ response [OR=0.138 (95% CI= 0.029, 0.654)]. In
this study we found that gender was the most consistent
predictor of response to TNF blocker therapy in
patients with polyarticular PsA, with females having a
lower probability ofresponse compared to males. These
findings suggest that gender-related biochemical, hormonal
and psychological factors could play an importantrole
in the response to TNF blockertherapy in PsA
